Getting sick is a humbling experience!

Again, I got a good reminder last week before I flew to Taipei to lead a session in an open source conference. I felt tired and powerless with a some fever and a lousy appetite. The feeling of hopelessness was unbearable. It was even probable that I have to cancel my trip Monday if I were not well enough or if I had high fever. You don't want to be quarantined in a foreign country when you cannot pass that thermal detection passage way ;-) in the airport. I checked the web on different symptoms of cold, flu, heat stroke and even the Avian flu.

It is probably interesting to have a quick comparison between a Chinese OTC (over the counter) drugstore and that in the United States. A typical Chinese OTC drugstore has actually a behind-the-counter setup. You walk in and tell them what your problems are, and they'll recommend a couple of things for you. You counter with some questions and you get a slip to pay the cashier and come back to pick up your medicine. These medicines can be completely composed of herbal ingredients or sometimes complete anti-biotics with its chemical name. Their names and labels tell you the basic what they'll do for you, but the more detailed information sheet is often *inside* the box.

In a typical "Walgreen" drugstore in California, you don't get advices normally since the people there are not trained to give any advices, and most places would not volunteer to do this because of possible liability issue. The setup is surely "over the counter", and you don't need to ask to see the package like the Chinese counterpart which are shelved behind the counter. I see one counter example in Yunnan when I was looking for a "Visine"-like eye drop early July. That store is more like a Walgreen store; you pick up what you need and line up at the checkout line. That store has very few choices as far as OTC medicines go. In this contrast, Chinese drugstore gives you the imagery of an old-fashioned apothecary in the West. I heard that modern-day French pharmacies are more like the Chinese ones as far as giving medical advices go.

I thought of this topic at the peak of my suffering in Beijing and finished this in Taipei. The pharmacies in Taipei is more like those in the US and you don't find as many Chinese medicines when compared with the ones in Beijing. The mainland Chinese did a lot of work in Chinese herbal medicine in the last 50-60 years.

One last note is that of the different attitudes towards 'cold' between Chinese and Americans. In the US, unless you have evidences of bacteria infections, it is typical to have the approach 'have good rest, take lots of liquid, and call your doctor in the morning if necessary'. In the Chinese setting, people tend to tackle the symptoms like fever and sore throat aggressively, and often at the expenses of using anti-biotics without much discrimination. Having symptoms making people uneasy and people rarely vieGetting sick is a humbling experience!w that those symptoms are just natural course of viral cycle which cannot be avoided, but can be contained and minimized. Psychiatric medication is a licenced psychoactive drug taken to exert an effect on the mental state and used to treat mental disorders. Usually utilized in psychiatric settings, these medications are made of synthetic chemical compounds, although some are naturally occurring.

Administration

Research

Prescription medication

A prescription drug is a licensed medicine that is regulated by legislation to require a prescription before it can be obtained. The term is used to distinguish it from over-the-counter drugs which can be obtained without a prescription. Different jurisdictions have different definitions of what constitutes a prescription drug.

Dispensation of prescription drugs often includes a package insert (in Europe, a Patient Information Leaflet or PIL) that gives detailed information about the drug.

In the United States, the Federal Food, Drug, and Cosmetic Act defines what requires a prescription. Prescription drugs are generally authorized by veterinarians, dentists, optometrists, physicians and nurse practitioners, though physician assistants do an increasing amount of drug prescribing under a physician's supervision. It is generally required that an MD, DO, DPM, NP, DVM, DDS, DMD, OD, or PA write the prescription; basic-level registered nurses (as opposed to advanced practice nurses such as a nurse practitioner, clinical nurse specialist, nurse anesthetist, and nurse midwife), emergency medical technicians, psychologists (but not psychiatrists, who are physicians), and social workers as examples, do not have the authority to prescribe drugs.

Unlike most other countries, the United States does not have governmental control of prescription drug prices, and US drug prices are usually viewed as very expensive in comparison to those countries who do. For those with health insurance, many health insurance programs (generally paid partially or in full by the patient's employer) have prescription payment plans where the patient pays only a small copayment and the pharmacy is reimbursed for the remaining cost by the insurance company using the premiums collected from all of the insured individuals and their employers. The uninsured typically must pay whatever higher drug price their local pharmacy charges. Prescription medication in United Kingdom


In the United Kingdom, a patient visits a general practitioner who is able to prescribe medicines. If given an NHS prescription, this can be taken to a pharmacy to be dispensed. District nurses and health visitors have had limited prescribing rights since the mid-nineties where prescription for dressings and simple medicines would have had to have been signed by a doctor. Extended prescribing was introduced in late 1999, where appropriately trained nurses could prescribe from a limited list of POMs. From 2006, some nurses and pharmacists will be permitted to prescribe all medicines in the British National Formulary, except controlled drugs directly. Each item on the prescription is liable to a prescription charge in England and Scotland of £6.85 (as of April 2007), although many patients are exempt from this charge. This includes those over 60, under 16 (or under 19 if in full-time education), patients with certain medical conditions and those on certain benefits. However in Wales prescription charges have been abolished - a situation the SNP government of Scotland hopes to emulate within the next four years.

An HC2 certificate can be applied for — although not automatically granted — by those on a low income or in receipt of incapacity benefit. Claimants of "Jobseekers" and "Income support" automatically receive free prescriptions and dentistry.

Those requiring regular prescriptions may make a saving by purchasing a pre-payment certificate which covers the cost of all prescriptions required for four months or a year. This charge is paid entirely to the NHS through the pharmacy, while the pharmacy claims the cost of the medicine dispensed. Each "item" can cover any prescribed item in a very large or very small quantity according to the doctor's prescription. This means that the patients perceived "value" of the charge varies enormously - the actual cost of the medicine given out will routinely vary from a few pence to hundreds of pounds.

The majority of items dispensed on NHS prescription are exempt from charges. This is because of the large number of medicines needed by, for example, the elderly or those with medical exemptions. NHS prescriptions can also be written for certain items by dentists and nurses. Some patients also receive private prescriptions, typically either from a doctor seen privately or for medicine not permitted on the NHS. For these, the patient will pay the pharmacy directly for the cost of the medicine and the pharmacy's markup.

Medical Testing is a Good Thing


Pharmaceuticals is a massive industry that at the last count was worth well in excess of $600 Billion every year. The industry is extremely highly regulated due to the dangers that are inherent in prescription medicines.

When new drugs appear on the market they don’t just happen to be there because they have some kind of effect on the symptoms that they are trying to alleviate. Before drugs can be sold or prescribed they have to undergo an extremely rigorous amount of testing. These tests can take many years to complete but they are mandatory due to the bodies ability to produce unwanted side effects when it comes into contact with new medicines.

You may have seen advertisements calling for ‘healthy volunteers.’ These people are wanted by the pharmaceutical testing laboratories to help evaluate what effects and possible side effects new medicines might have on the human body.

Before drugs get to these stages they are tested extensively in labs on rats, mice and guinea pigs. However, there is still sometimes unwelcome problems when testing on human patients for the first time.

You may remember a news story about some people taking part in a drugs trial in the UK in 2005. They were given intravenous drugs that produced terrible swelling of the whole body, particularly the head and hands. All of the people survived what would have been a very scary experience. Only the very best pharmaceutical recruiters would have been able to recruit for new volunteers for that test.

We also work with other medical resources in the States and Honduras when available to facilitate and coordinate their help to this area. The clinic also assists in collection of medical statistics to aid program development.

Medical supplies are always needed. And some local staff is used to contact patients to return to the clinic for treatment of chronic medical needs like diabetes or high blood pressure. Patients pay for the medicine when they are able; if they cannot pay for it, it is given without cost. Donations of medicine, basic medical supplies, and funds help sustain this effort. Physician services available at the clinic are provided at no cost to either donors or patients.

The clinic has been in operation for several years. This continuing operation has been much more effective than many efforts that only operated for short periods of time before being abandoned.

Sometimes simple medical items like Tylenol, vitamins, and supplements are really very helpful. This shipment was donated by someone in the US.

Traditional Chinese Medicine History

The history of traditional Chinese medicine can be traced through archaeological excavations extending back millions of years.

Primitive people spent most of their time on basic survival: hunting, locating and preparing plants for food, building shelters, and defending themselves. It's easy to imagine that over time, they'd have sampled most of the local plants in their search for food. In time, an oral record evolved that identified those plants that made good food, those that were useful for building, those that had an effect on illness, and those that were poisonous. Through trial and error, a primitive form of herbal medicine and dietary therapy was taking shape in China.

Fire also played a central role in their lives as a source of warmth, fuel, and light. As they huddled around fires, it was only natural that our ancestors would discover the healing powers of heat. Those powers would have been especially evident for cold, damp ailments such as arthritis, for which heat provides immediate relief. This was the origin of the art of moxibustion, the therapeutic application of heat to treat a wide variety of conditions.

These ancient people must have experienced a variety of injuries during their rugged lives. A natural reaction to pain is to rub or press on the affected area. This hands-on therapy gradually evolved into a system of therapeutic manipulation. People discovered that pressing on certain points on the body had wide-ranging effects. They began to use pieces of sharpened bone or stone to enhance the sensation, and acupuncture was born.
